Minor Modifications
At Home Assist Community Services, we are proud to offer Home Modifications services under the Commonwealth Home Support Programme (CHSP) to eligible consumers in the regional areas of Bundaberg, Fraser Coast, North Burnett, and South Burnett.
Our aim is to assist individuals in creating safe and accessible living environments within their own homes. Please note that our services are subject to funding and eligibility requirements and wait times do apply.
Materials are paid by the consumer and a quote will be provided prior to work commencing.
Minor Home Modifications
Our Minor Home Modifications service focuses on making minor structural changes to enable individuals to live and move safely around their homes. Some of the modifications we provide include: Occupational Therapy Assessment and referral codes provided by My Aged Care for
- Fitting grab rails for added stability
- Installing small ramps or step modifications
- Adapting tap sets for easier use
- Installing hand-held showers
- Other safety and mobility aids as needed
Consumer eligibility for Home Modifications services is determined by My Aged Care.
Once eligibility has been established, My Aged Care will arrange for an Occupational Therapist Assessment and provide a referral to Home Assist Community Services. Alternatively, My Aged Care may provide you with a referral code. Following the Occupational Therapist Assessment, the assessing Occupational Therapist will send a referral to Home Assist Community Services, enabling us to proceed with the Home Modifications request.
