Home Assist Community Services partners with individuals to coordinate a tailored package of care and services that align with their assessed needs, goals, and preferences, allowing them to remain living at home for as long as possible. The Home Care Package Program supports older Australians with complex ageing-related care needs through a consumer-directed approach, helping them maintain their independence and capabilities as they age. These packages provide coordinated care and services within the individual’s home care budget and the program’s scope, ensuring support is personalised and respectful of each person’s circumstances. The program offers a government-subsidised package, with additional supplements available for eligible individuals, to deliver the necessary care, services, and management required to meet their assessed needs. With four package levels available, the program caters to varying levels of care needs, ensuring people can safely remain in their homes for as long as it is appropriate.

Social Support

Helps seniors stay engaged, independent, and connected with their community. Whether it’s preparing grocery lists, reading mail, enjoying friendly conversation, or playing cards, our team is here to provide companionship and assistance. We also offer help with errands, transportation to appointments and social outings, and both accompanied and unaccompanied grocery shopping.

Personal Care

Support daily tasks like bathing, grooming, dressing, and mobility assistance. We also help with toileting, dining, exercises, and communication needs, including hearing aids and spectacles. With a focus on dignity and independence, our team provides personalised care to enhance your comfort and well-being.

Respite Services

Provides short-term care, allowing family careers to rest or attend personal activities while their loved one remains safe and comfortable at home.
For extended support, Overnight Respite offers 12 hours of coverage, including four active care hours and an eight-hour sleepover period. Additional personal care hours can be arranged at an hourly rate.

Yard Maintenance

Help keep your garden and outdoor areas safe, accessible, and well-maintained. We assist with basic lawn care, pruning, weeding, and general upkeep to ensure a clean and hazard-free environment.

Nursing Care

Nursing service provides care for consumers on a daily, weekly, monthly, or as needed basis as ordered by your primary care provider.

Domestic Assistance

Provides eligible consumers with a clean focused on health and safety. We assist with tasks such as carpet cleaning, mold removal, windows, ceilings, ovens, and more. Cleaning products and materials are the responsibility of the consumer, and wait times may apply based on funding availability.

Allied Health

Home Assist provides a range of Allied Health Services. Whether your goals are learning new skills, improving your health or quality of life, our Allied Health Therapists can support you along the way.

Home Modifications

Help individuals live safely and independently at home. Minor modifications include essential safety improvements such as grab rails, small ramps, step adjustments, and handheld showers. Major modifications offer subsidised solutions like wheelchair-accessible bathrooms, ramps, and stair climbers. These require a specialised assessment and approval.

Home Maintenance

To assist those who need help with various household tasks and minor repairs. Whether it’s fixing small issues around the home, installing safety features, or handling general upkeep, our skilled contractors are committed to keeping your home safe, functional, and well-maintained.

Clinical Care

There are some care services which can only be provided by nurses. If you have an injury, illness, or aged care need which requires regular clinical attention in-home nursing care is the perfect way to rest and recover.

Goods, Equipment & Assistive Technology

We provide a range of aids and equipment to support mobility, safety, and independence at home. This includes walking aids such as crutches, walking frames, and wheelchairs, as well as mechanical devices like bed rails, slide sheets, and pressure-relieving mattresses. Personal monitoring alarms and Telehealth support services are also available to enhance safety and access to care.
